The Application form is in a PDF format and can now completed
online. After completion, it can be printed for mailing.

When you have completed the form, click on the "Print" button
and the completed form
will be printed. Mail the competed form, along with your dues,
to Prim Hamilton at the address on the form.
Directions for completing the form: When the form is opened, there should be a line of text at the
top of the screen that says the form is interactive - to the
right of this line is a box for "highlight fields." If
the fields are not already highlighted, simply check the box and all the interactive fields will be highlighted.
Click in the first field to start
entering information. Tab to move from field to field, Shift/Tab
to move backwards (or just click in a field box).
With the exception of the Association Mailing Address field, all
the other address fields will allow you to put in two lines
if you do not have enough room.
